C’est un nouveau bilan sur la durée possible des symptômes du COVID-long, qui vient d’être réalisé par une équipe internationale de scientifiques d’Islande, de Suède, de Norvège, du Danemark, d’Estonie et du Royaume-Uni. Un bilan qui révèle que si la plupart des symptômes de santé mentale chez les personnes qui récupèrent du COVID-19 disparaissent dans les 2 mois suivant le diagnostic, chez les patients atteints de formes plus sévères de COVID-19 et alités pendant 7 jours ou plus, le risque de dépression majeure et de troubles anxieux est accru de 50 %, 16 mois plus tard.

C’est l’une des premières études à examiner les implications à si long terme pour la santé mentale des patients ayant développé une forme sévère de COVID-19, à partir d’un si grand nombre de données de plusieurs pays. À ce jour en effet, la plupart des études n’ont examiné les effets néfastes sur la santé mentale que jusqu’à 6 mois après le diagnostic de COVID-19, et on connait encore peu les effets à long terme sur la santé mentale au-delà de cette période, en particulier pour les patients non hospitalisés et présentant divers degrés de gravité de la maladie.

Ce bilan, outre une rémanence longue de l’anxiété et de la dépression chez les patients diagnostiqués de formes plus sévères, suggère que ce constat ne révèle que « la partie visible de l’iceberg » : les patients diagnostiqués d’une forme légère étant moins susceptibles de souffrir de dépression et d’anxiété que ceux qui n’ont jamais été diagnostiqués.

L’étude a évalué précisément la prévalence des symptômes de dépression, d’anxiété, de détresse liée au COVID-19 et de mauvaise qualité du sommeil chez les personnes avec et sans diagnostic (suivi moyen de 6 mois). Au total, l’analyse a porté sur les données de 247.249 participants dont 4 % (n=9.979) ont reçu un diagnostic de COVID-19 entre février 2020 et août 2021. L’analyse conclut, dans l’ensemble, que :

  • les patients non hospitalisés atteints d’une infection par le SRAS-CoV-2 sont plus susceptibles de présenter des symptômes dépressifs jusqu’à 16 mois après le diagnostic -que ceux qui n’ont jamais été infectés ;
  • les patients alités pendant sept jours ou plus -soit un critère de forme plus sévère- présentent des taux plus élevés de dépression et d’anxiété, par rapport aux patients diagnostiqués mais jamais alités ;
  • les symptômes de dépression et d’anxiété s’atténuent généralement en 2 mois pour les patients non hospitalisés atteints de COVID-19.
